Helping Individuals With Special Needs Improve Their Well-Being

Our adult foster care provides a safe and caring environment for older adults and people with disabilities. Here at P&N Vision HealthCare Services, Inc., our adult foster care program aims to let people experience compassionate care, achieve financial assistance, and gain resources to allow families to thrive more in life. We believe this program best fits those who live alone and may have a tough time caring for themselves, which requires a full-time caregiver to cater to their needs.

Our full-time caregivers can provide the following:

  • Hands-on assistance with toileting, bathing, feeding, ambulation, and other activities of daily living (ADLs)
  • Behavioral Management
  • Round-the-clock care and supervision

The services we provide are specifically tailored to meet the needs and preferences of individuals. We have dedicated and compassionate case managers and registered nurses who partner with families to help care for their family members who are enrolled in foster care. Our staff members come from different backgrounds and speak many languages, such as English, Swahili, and Spanish.

We provide support to caregivers and their loved ones through:

  • A tax-free stipend
  • Being a listening ear
  • Participation in other programs and services
  • Referrals to services and community resources
  • Regular visits and assessments done by a registered nurse or case manager

Eligibility

For Individuals Supported

Here is a list of eligibility requirements for individuals receiving care under the AFC program:

  • Be 16 years or older
  • Have a doctor’s order stating a medical, mental, and/or physical need
  • Live with a caregiver in a private home in Massachusetts
  • Qualified for MassHealth or a version of a Senior Care Options (SCO)
  • Require assistance with ADLs, personal care, and behavioral support

For Caregivers

Here is a list of eligibility requirements for individuals receiving care under the AFC program:

  • Be at least 18 years old
  • Follow AFC regulations
  • Live with the person who needs assistance
  • Not a spouse, parent of a minor, or legal guardian of the person in need of care
  • Pass a CORI (Criminal Offender Record Information) check

Note: The caregiver may or may not be a family member.
